&lt;div class="ed_title01">Reissuing new cards to staff members&lt;/div>&#xd;

&lt;div class="ed_txt">New staff members can apply for a card at the Information Technology Service Center one day after reporting to the personnel department. Bring your contract form or ID to verify your identity when applying for the card.&lt;/div>&#xd;
&#xd;
&lt;div class="ed_list">&#xd;
&lt;ul>&#xd;
	&lt;li>Bring your ID card and an electronic file of your ID photo (JPG file) to the Information Technology Service Center.&lt;br>&#xd;
	Photo specifications: &lt;strong>200 (W) × 250 (H) pixels. The name of the file&lt;/strong> should be your &lt;strong>personnel number&lt;/strong>.&lt;br>&#xd;
	&lt;strong>*Note:&lt;/strong> The Information Technology Service Center &lt;strong>will NOT replace unsatisfactory photo results&lt;/strong> &lt;strong>on the personnel ID card FOR FREE&lt;/strong> if the problem is caused by &lt;strong>incorrect photo specifications&lt;/strong>.  &lt;/li>&#xd;
	&lt;li>Existing staff members should bring their personnel ID card to verify their identities when applying for a new card.&lt;/li>&#xd;

&lt;div class="ed_title01">Campus IC Card Replacement/Exchange Application&lt;/div>&#xd;
&#xd;
&lt;div class="ed_txt">Faculty or student ID cards requiring replacement due to loss or malfunction can be applied for using the following steps:&lt;/div>&#xd;
&#xd;
&lt;div class="ed_list">&#xd;
&lt;ol>&#xd;
	&lt;li>Log in through the campus system's single-entry portal.&lt;/li>&#xd;
	&lt;li>From the [National Yang Ming Chiao Tung University] system list, select [New LossReport System for Campus ID Card]. After entering, navigate to the [Loss refund] page in the left menu and click the [Loss refund] button.&lt;/li>&#xd;
	&lt;li>After completing the online application, click [Print Replacement Application Form].&lt;/li>&#xd;
	&lt;li>Take the replacement application form to the Cashier's Office or use an automatic payment machine to pay the fee. Alternatively, pay on-site via mobile payment when applying for card replacement at the Information Center (using JKO Pay).&lt;/li>&#xd;
&lt;/ol>&#xd;
&lt;strong>&lt;span style="font-size:110%;">Issuance of a New Card&lt;/span>&lt;/strong>&lt;br>&#xd;
Bring the replacement application form (with receipt) and identification documents to the Information Center service desk to process the issuance.&lt;br>&#xd;
&lt;strong>&lt;span style="color:#e74c3c;">Important Reminder: Once the loss reporting operation is completed, it cannot be revoked. Please confirm carefully before proceeding.&lt;/span>&lt;/strong>&lt;br>&#xd;
